  • Citizens Insurance Company of America, Howell, Mich.; fine $75,000 / fine reason: the insurance company charges incorrectly on 135 policies, and fills in the forms on 7 policies incorrectly.
  • Mutual of Omaha Insurance Co., Omaha, Neb.; Fine $12,000, /Reason for fine: In 2016 and 2017, the insurance company spent $98,400 to reward 17 brokers who met the health insurance and life insurance performance standards to travel at public expense.Washington State law prohibits such behavior.
